Our Services

Hospice of the Comforter’s services are provided regardless of your ability to pay and are provided in the comfort of your home. Our individualized care plans ensure that you and your loved one receive the education, support and compassionate care you need. Patients and caregivers also benefit from 24/7 telephone support. We are here for you.
As you receive hospice care, you and your loved one will lead our team and provide day-to-day care with the support of Hospice of the Comforter. Your primary physician may continue to manage your care along with our hospice team.
Your Hospice of the Comforter care team will provide the following services:
Hospice Physician
- Visits patients, coordinates care and works closely with the hospice team to provide care and manage symptoms
Patient Care Manager
- Manages your team and serves as your primary contact
Nurse Case Manager
- Coordinates care with the physician and your hospice team
- Ensures the patient is as comfortable as possible through pain and symptom management
- Teaches patients and caregivers
Social Worker
- Provides emotional support and counseling
- Connects families to community resources as needed
- Assists with Advance Directives such as a Living Will
Chaplain
- Offers non-denominational spiritual support
- Assists with funeral service arrangements if requested
Certified Nursing Assistant
- Provides personal care such as bathing or shaving
Trained Volunteer
- Provides a variety of services such as patient companionship and caregiver respite
Bereavement Counselor
- Keeps in touch with the family for up to 13 months
- Offers individual and group grief support
Additional services
You will experience improved comfort and safety with our additional services such as medications, medical equipment and disposable supplies. When related to your hospice diagnosis, there is no cost for these additional services and they can be delivered directly to your home.
Medications
Hospice of the Comforter provides medications related to your hospice diagnosis for comfort and symptom management.
Medications not covered will need to be obtained through your regular pharmacy.
Your nurse case manager will let you know which medications will be covered.
Medical Equipment
Hospice of the Comforter provides medical equipment such as electric beds, walkers, over-the-bed tables and wheelchairs to promote comfort and safety.
Disposable Supplies
Hospice of the Comforter provides supplies such as gloves, adult briefs and wound and skin care supplies to promote comfort and safety.
